About this performance review form

A performance review looks back at a period (usually a year or six months), records what was achieved against goals, rates key competencies and agrees goals and development for the next period. A consistent form makes reviews fairer and gives both sides a written record.

This template includes a goals review table, ratings for six common competencies, strengths and development areas, next period’s goals, an overall rating and space for both the manager’s and the employee’s comments.

What’s on the performance review form

The template has 5 sections and 22 questions. Every one can be renamed, moved or deleted.

Tips

  • Ask the employee to fill in a self-review first, then compare in the meeting.
  • Base ratings on examples from the whole period, not just the last month.
  • Write goals that are specific and measurable, with a date.

Four ways to save it

Should the employee sign the review?

Yes. Signing confirms they’ve seen it, not that they agree; the employee comments box lets them record disagreement.
