Each term, schools ask students which after-school or co-curricular activities they want to join. An extracurricular activity form lists the options, asks for first, second and third choices, checks transport and timing, and gets a parent’s permission and agreement to any costs.

Students can also record activities for their portfolio, which helps with later applications.

What’s on the extracurricular activity form

The template has 4 sections and 13 questions. Every one can be renamed, moved or deleted.

• Activities offered: Activities this term (tick all that apply: Football, Basketball, Cricket, Athletics, Swimming, Music, Dance, Drama, Art, Robotics, Coding, Chess, Debate, Eco club); First choice; Second choice; Third choice
• Practical details: Days my child can stay after school (tick all that apply: Mon, Tue, Wed, Thu, Fri, Sat); Going home after activities (choose one: Collected, School bus, Walks, Other); Medical needs for sport or activity
• Permission: Agreement: “I give permission for my child to take part and agree to any costs shown.”; Parent or guardian signature with date

Tips

• List each activity with its day, time and cost in the options.
• Ask for ranked choices so you can balance numbers.
• Confirm placements in writing before the first session.
