About this peer evaluation form

Group projects work best when everyone contributes, and peer evaluation is how teachers find out who did. Each student rates every teammate (and themselves) on contribution, reliability, communication and quality of work, with a comment. The teacher can then adjust individual marks fairly.

Tell students about peer evaluation at the start of the project; it changes behaviour more than any reminder.

What’s on the peer evaluation form

The template has 3 sections and 7 questions. Every one can be renamed, moved or deleted.

  • Project: Your name; Project; Group; Date
  • Ratings (1 = poor, 5 = excellent): Ratings table (Team member, Contribution, Reliability, Communication, Quality, Comment)
  • Reflection: What went well in our group?; What would you do differently?

Tips

  • Keep evaluations confidential so students answer honestly.
  • Look for patterns across the group, not one low rating.
  • Pair it with a rubric for the project itself.

Four ways to save it

A fillable PDF has typing boxes, tick boxes and choices built in. A printable PDF gives clean boxes and writing lines for pen and paper. How many teammates can the form handle?

The table has five rows. Change the number of rows for larger groups.

Can I add my logo to the peer evaluation form?

Yes. Add your organisation’s name, a contact line and a logo in “Form details”. The logo is placed into the file on your device and is never uploaded.

Can people fill in this peer evaluation form on a phone?
