About this purchase requisition form

A purchase requisition is an internal request to buy something. The person who needs the item fills it in, a manager or budget holder approves it, and only then does purchasing raise a purchase order with the supplier. It’s how organisations keep spending within budget and avoid duplicate orders.

The template lists items with quantities and estimated prices, a suggested supplier, the budget or cost centre, why it’s needed and when, followed by approval and purchasing sections.

What’s on the purchase requisition form

The template has 5 sections and 15 questions. Every one can be renamed, moved or deleted.

  • Requested by: Name; Department; Date; Needed by; Budget or cost centre
  • Items: Items table (Item and description, Qty, Unit, Est. unit price, Est. total); Estimated total
  • Supplier and reason: Suggested supplier; Quotes attached (choose one: None, 1, 2, 3 or more); Why is this needed?

Tips

  • Attach quotes for larger purchases; many policies need two or three.
  • Use a budget or cost-centre code so finance can allocate the cost.

What’s the difference between a requisition and a purchase order?

A requisition is internal: “please buy this”. A purchase order is external: the official order sent to the supplier after the requisition is approved.

Who approves a purchase requisition?

Usually the department head or budget holder, with higher amounts needing finance or director approval. Write your approval limits in the instructions.

Can I add my logo to the purchase requisition form?

Yes. Add your organisation’s name, a contact line and a logo in “Form details”. The logo is placed into the file on your device and is never uploaded.

Can I get the purchase requisition form as a Word document?

Yes. Choose “Word” under “Save as” to download a .docx file you can keep editing in Microsoft Word, Google Docs or LibreOffice.

Is anything I type sent to a server?
